About Linea

\n

Linea is building a scalable, agentic-led transitions-of-care program for patients who have just left the hospital. For 90 days after discharge, we find and close the gaps that send people back: missed follow-up, medications that were changed but never filled, and social barriers like transportation, food, housing, and cost.

\n

\n

We are breaking the mold of traditional transitional care. The usual model puts a nurse or coordinator on the phone with a small caseload, chasing callbacks and faxes, acting as a middleman between the patient and their doctors. It does not scale, and the patient's own voice often gets lost along the way.

\n

\n

Automation lets patients speak up. Our automated check-in calls and texts reach every patient on a regular cadence and ask what has changed. Patients raise their needs in their own words: a prescription they cannot afford, an appointment no one scheduled, a symptom that worries them. Our agents turn those needs into tasks, with the patient's words attached, so the gap is clear from the start.

\n

\n

We enhance care teams; we do not replace them. The patient's PCP, cardiologist, and nephrologist stay in charge of their care. Our team is the accelerator for expert communication. We take what the patient told us, confirm it against the chart, and get a clear, professional message to the right clinician at the right time.

\n

We started in heart failure and chronic kidney disease, including dialysis, and are expanding across the cardio-renal-metabolic space and beyond. We work under shared-risk contracts with ACO partners, so outcomes are what we are measured on.

\n

\n

The Role

\n

We are looking for Care Navigators with a clinical or medical background who want to put that knowledge to work after a patient leaves the hospital. You will help patients and their care teams close the gaps that lead to readmissions: missed follow-up, medication changes that never reached the patient, and social barriers that make a care plan hard to follow.

\n

You will work alongside our AI tools and clinical team, reaching patients by phone and text and communicating clearly and professionally with their doctors and care teams. This is a non-clinical role: the patient's own clinicians make every clinical decision. Your background helps you recognize what matters, explain it clearly, and make sure it reaches the right person.

\n

\n

How We Work

\n
    \n
  • Technology handles the routine work. Our AI agents take on routine outreach and surface patient needs, so your time goes to the conversations that need a person.
  • \n
  • The patient's voice comes first. We listen to what patients tell us and make sure their care team hears it.
  • \n
  • We work with care teams, not around them. You will communicate with physician offices and other providers clearly and professionally.
  • \n
  • Small team, real ownership. We are an early-stage company, and our navigators help shape how the work gets done. We walk through the details of our process during interviews.
  • \n
\n

\n

Key Responsibilities

\n
    \n
  • Support patients after hospital discharge by phone and text, helping them understand their follow-up plan and next steps
  • \n
  • Identify gaps in follow-up care, medications, and social needs, and communicate them clearly to the patient's care team
  • \n
  • Coordinate with physician offices, pharmacies, and community organizations to close those gaps
  • \n
  • Call emergency department teams when a patient returns to the ED to offer care coordination and to help prevent an unnecessary readmission
  • \n
  • Connect patients with resources for transportation, food, housing, and medication costs
  • \n
  • Keep accurate, concise documentation of patient interactions
  • \n
  • Work alongside AI tools and share feedback that helps improve them
  • \n
  • Collaborate closely with Linea's clinical team
  • \n
\n

\n

Qualifications

\n

Required

\n
    \n
  • At least 2 years of clinical experience, ideally in internal medicine, cardiology, nephrology, emergency, or hospital medicine
  • \n
  • Working knowledge of, or familiarity with, heart failure, kidney disease, dialysis, and diabetes, including common medications
  • \n
  • Excellent written and spoken English, with the confidence to speak with physicians, nurses, and office staff
  • \n
  • Comfort working with technology and AI tools
  • \n
  • Strong organization and follow-through
  • \n
  • Authorization to work in the United States
  • \n
\n

\n

Preferred

\n
    \n
  • Medical or nursing degree (MD, MBBS, RN, or equivalent), including training completed outside the US
  • \n
  • Experience in care coordination, case management, or transitions of care
  • \n
  • Advanced master's degree in public health, social work, or other interest in population health
  • \n
  • Familiarity with the US healthcare system
  • \n
  • Availability to work Pacific time hours through 5 PM PT, with flexibility to work weekends and holidays
  • \n
  • Bilingual (Spanish or other languages)
  • \n
  • Telephonic or telehealth experience
  • \n
\n

\n

Why Join Linea

\n
    \n
  • Put your background to work. Use your clinical knowledge every day to find gaps and explain them to US clinicians, clinician to clinician.
  • \n
  • Reach more patients. Agents handle routine outreach so your time goes to the patients and problems that need a person.
  • \n
  • Shape the model. Your feedback changes what the platform and agents do. We are building this with our navigators, not around them.
  • \n
  • Real outcomes. Every week you will catch a missed medication, a missed appointment, or a social barrier before it becomes a readmission.
  • \n
  • Growth. Build US healthcare experience in population health, transitions of care, and AI-enabled care delivery at an early-stage company.
  • \n
\n

Package. Salary, remote work, and eligibility for a discretionary performance bonus.

\n

\n

Application Process

\n

Submit:

\n
    \n
  1. Your resume, including your clinical background
  2. \n
  3. A short cover letter answering: (a) a time you found a gap in a patient's care that others missed, (b) how you would explain an unfilled heart failure medication to a busy cardiology office in three sentences, and (c) why working alongside AI agents interests you
  4. \n
\n
